Application Procedures For Medical Cases

Applicants for medical visas must apply for a regular B1/B2 visa. All visa applicants are required to complete the application forms DS-156 and DS-157 online, then print them and bring to the interview. Both forms can be found at: http://travel.state.gov/visa/frvi/forms/forms_1342.html

All non-immigrant visa applicants applying on or after March 1, 2008 will be required to schedule appointments for a visa interview online.   Applicants may do so by visiting the following website: https://evisaforms.state.gov/default.asp?postcode=TBL&appcode=3

Please note that relatives or friends that wish to travel in support of a medical visa applicant will have to apply and qualify for a regular tourist visa (B1/B2). For an appointment please follow the directions listed on the following page:

Please bring the following to your interview:

• your passport (valid at least another 12 months)
• $131 non-refundable application fee.
• the completed visa applications DS-156 and DS-157
• 1 unmounted 5 x 5cm, full face photograph stapled or glued to the application form

Documentation Needed When Seeking to Travel for Medical Treatment

Persons desiring to travel to the U.S. for medical treatment will be required to present the following, in addition to the documentation stated above and any other that the consular officer may require:

1. Letter of medical diagnosis from a local physician, explaining the nature of the ailment and the reason the applicant requires treatment in the United States.
2. Letter from a physician or medical facility in the United States, expressing a willingness to treat the ailment and detailing the projected length and cost of treatment (including doctors’ fees, hospitalization fees, and all medical-related expenses).
3. Evidence of ability to pay for the specific treatment plus anticipated associated costs (such as transportation, outpatient treatment lodgings, and followup therapy), and information on sources of financial assistance (where appropriate).

Please note that applicants who plan to go to the United States for medical treatment are required to meet all statutory eligibility requirements for a nonimmigrant visa.
