ADOPTION VISA DOCUMENTARY REQUIREMENTS

Panel Physicians

  • Note: Americans who are adopting a child in Azerbaijan are welcome to use the panel physicians in Baku to conduct the required medical examination for the child's immigrant visa application. Please provide the consular officer at the American Embassy in Baku with the medical exam results in a sealed envelope as part of the Request for Orphan Investigation (I-604). The I-604 Orphan Investigation must always be conducted in the country where a child was adopted. The consular officer at Embassy Baku will place the sealed medical exam results into another sealed enveloped containing the results of the I-604 Orphan Investigation which should be hand-carried to the immigrant visa interview at the American Embassy Tbilisi.
