Americans should safeguard passports as they would any other valuable.
In order to issue an American passport, we must have proof of identity and citizenship. If you are renewing a valid or recently expired passport, your passport will serve as proof of both identity and citizenship. Otherwise to prove your identity, you need to provide two forms of photo ID, such as a driver's license. To prove citizenship, you need to provide a certified copy of your birth certificate, certificate of naturalization or other similar documents. All passport applicants must provide two identical 2" x 2" photos in either color or black and white with a white background.
FOR MINORS: Both parents, along with each minor, must be physically present in the Consular Section to sign a minor's passport application. If one parent is not physically present, then notarized form DS-3053 should accompany the application. If the missing parent is not in the

If unable to submit a passport with the required documents, applicants must submit form DS-64, Statement of Lost or Stolen Passport, a written statement describing the circumstance under which the passport was lost or stolen. In this case, adult applicants should complete form DS-11 instead of DS-82 and pay an additional $25 execution fee.

American citizens residing or traveling abroad who require issuance of a U.S. passport are issued the latest, state-of-the-art passport incorporating a photo-digitized image and other enhanced security features. U.S. Embassies and Consulates achieve this goal by transferring the passport issuance function to a domestic U.S. passport facility. This has increased processing times, but the Department of State is committed to ensuring that American citizens receive secure documents in a timely manner. American citizens are encouraged to apply early as it takes at least two weeks processing time.
In the event of a true emergency, the consular section can still issue limited validity passports, which cannot be extended. Bearers will be required, however, to exchange their limited validity passports for a full-validity photo-digitized passport upon completion of their emergency travel, both through passport facilities in the U.S. and U.S. Embassies and Consulates abroad. Only one fee will be assessed in such cases.
Current fees are $75 for adult renewals, $100 to replace adult lost and stolen passports, and $85 for all minor (under 16) passports.

Department institutes no-amendment policy:
Effective September 26, 2005 the Department will no longer amend passports. Individuals who need a name change or correction of bio data, or who seek to extend a limited validity of passport will need to apply for a replacement passport. If the change is requested within one year of the passport issuance date, there will be no charge for replacement service. Changes requested more then one year after the original issuance date, however, will require the applicant to pay the full passport renewal fee. Visa pages will continue to be added free of charge. This change is an additional security enhancement for the passport and one more step toward further securing the integrity of the U.S. passport.
